No premium participation for regular full-time employees. The cost to cover eligible family members is $150.00 a month. Eligible dependents are those up to the age of 26 without other employer coverage regardless of residence, student status, marital status or financial dependency. Effective on the first day of the calendar month following 30 days of employment.
